Monthly Governance Report · Month 3, June 2026

A clean month, and two actions to close

June is the strongest month to date: all three packs met their on-time targets, volume rose 12% on May, and there were no governance breaches or data incidents. Action statuses are shown for the 6 July review, when two actions are overdue; both await records from outside the skills team and are recommended for a hard close.

02Pack performance

How each pack performed in June

June rates and quality scores are independently supplied illustrative metrics. Their denominators, weighting and assessment basis are not provided; totals are not recalculated from prepared counts. Margin Cockpit has the highest scores; Commercial Control Kit's quality gap is attributed to late site-diary uploads.

Where the quality gap sits

Commercial Control Kit quality at 89% is one point below target. The root cause is three evidence bundles returned for missing supporting documents, traced to site teams not uploading diary extracts within the agreed window. A reminder went to all site managers through T. Brennan.

Review gates and open actions

All six June review gates passed. Every item was checked by its preparer before approval. The separate illustrative 91% first-pass measure has no supplied calculation basis. All four weekly summaries were on time; no unauthorised access or escalation events were recorded. All 87 outputs were logged by named Greystone users in the agreed environment. Action statuses below are at 6 July.

Escalation to the sponsor

At the 6 July review, A-017 and A-019 are overdue and depend on records from outside the skills team. Recommend escalation to D. Farrow, with a proposed hard close date of 13 July to prevent recurrence.

04Recommendations

Recommendations and July

Four recommendations: close the two overdue actions by 13 July with named owners; progress the 24 July RAMS assessment following recurring mobilisation delays across three contracts; add a site-team data-readiness checklist before Monday's records cut-off to address Commercial Control Kit quality; and either move morning handover to 07:45 or record the Harland BMS feed as a formal dependency.
