No generic copy-paste controls: every control is engineered for the task it sits against and the residual risk it has to bring down. Thirty-three hazards across eight tasks, twelve of them initially high or very high, none of them high or very high once controls are in place.

Task-specific, not templated
Hazards are generated from the task sequence and the job intake against a task-hazard knowledge base, then reviewed by a competent person before issue. Likelihood times severity gives the risk score on a 5 by 5 scale. Initial scores show the inherent risk of each hazard; residual scores show how far the controls bring it down. The matrix is the baseline; dynamic risk assessment runs daily and the matrix is reviewed if scope, sequence or programme change.
The illustrated matrix records twelve initially high or very-high hazards and a maximum medium residual after the proposed controls. These scores are for review, not an accepted threshold for occupied-building Cat B fit-out; a competent person must verify the controls and assess acceptability before issue.
Eight tasks, hazards attached to each
Hazards are attached to the task that generates them, not carried across the whole scope. The count against each task is the number of individually assessed hazards.

Thirty of the thirty-three residuals sit at low; the remainder at medium. None remains high or very high.
This matrix remains for approval by a competent person; it does not authorise work. The three medium residuals, asbestos disturbance, adjacent live electrical and hidden services, keep their permit and sign-off controls for the life of the works, and the matrix is reissued if the sequence or the occupancy changes.
