Every question raised during the talk is captured here for the record. If a question can't be answered on the spot, it's escalated and the talk is paused until the answer is on hand.
The certificate is the formal record that the toolbox talk was delivered and understood. It carries the signature of the briefer and is referenced in the project's audit trail (Output 02 — HRC Matrix). A copy is filed in the site safety dossier and a copy retained by the Principal Contractor.
The briefing covered working at height, mortar dust silica risk, lime burns, falling debris, heritage damage and emergency arrangements. Seven operatives attended. Four questions were raised and answered. All signatures captured before any operative ascended the scaffold.
